“Policy on the Handling of Personal Information (Privacy Policy)”
We implement the procedures for appropriately and securely protecting private information by establishing a private information protection and management system according to the policy mentioned below to properly protect and mange the private information of our customers, business partners, and employees.
- 1.Acquisition, use, and supply of private information
- When we acquire private information, we do so by specifying and explicitly disclosing the purposes and by obtaining the consent of the party providing the information to the extent necessary to achieve our business purposes.
The private information acquired shall be used and provided within the limitation of usage specified and shall not be used beyond the limitation necessary to achieve our business purposes without the consent of the party providing the information. We also take strict measures so that the information will not be used beyond its purpose for use. - 2.Safety management measures
- We implement strict safety management measures so that the information acquired may not be accessed without authorization, leaked, lost, or damaged. The measures shall be reviewed regularly for improvement on a constant basis.
- 3.Handling complaints
- We establish a dedicated section to sincerely and swiftly handle any complaints, consultation, and inquiry from the informants about the information they provided.
- 4.Regulatory compliance
- We shall comply with any law, guidance set by the government, and other rules and regulations regarding the protection of private information including the “Act on the Protection of Personal Information” while building a private information protection and management system based on the requirements JISQ15001 to observe them.
- 5.Regular improvement
- We shall review our private information protection and management system regularly and make improvement on a constant basis.

- We shall not provide the third parties with the private information we acquired without the consent of the informants.
- 3.Provision of private information
- In case we outsource our businesses to a third party for the purpose of completing the purpose of the information usage, we shall select a outsourcing company sufficiently satisfying private information protection criteria and by executing a private information protection agreement with them.
- 4.Discretion on private information
- The informant has the right to decide the existence of private information requested. However, it should be noted that in case the informant is not able to provide necessary private information, we may not provide the informant with the answers to their inquiries or part of our services.
- 5.Risks and indemnification
- While the information sent through our online application form is encrypted by SSL, we are unable to rule out the possibility that the information may be monitored and leaked when we receive it with our server. Should the information sent be monitored or leaked, we shall not be responsible for any damaged caused.
- 6.Informant’s exercise of their right on private information
- We will handle any request to notify, disclose the purpose of using private information, to correct, add, or delete the content of the information, to terminate and delete its usage, and to terminate its provision to third parties by the informant (or their agent) swiftly according to our pre-specified procedures.

In case your agent is applying on your behalf, please enclose the following documents as well as the documents mentioned above.(1)In case the agent is a statutory agent (the person with parental authority or the guardian of adult), one of the documents listed below needs to be submitted to prove that you are a real statutory agent.

2.A copy of medical insurance certificate listing all the dependents
3.Certificate of registered information
(2)In case the agent is assigned by delegation, a copy of power of attorney stamped with registered personal seal along with your certificate of a seal impression.

■ Fees
For your request of disclosure, please pay the following fees every time you make a request. While the payment needs to be made by mailing a money order, you may pay in cash when you visit us.
1)Disclosure: 1,000 yen
2)Notice of usage purpose: 500 yen (not applicable when you make this request at the same time you make a disclosure request.)
3)Correction, addition, deletion, termination of usage, deletion, and termination of the provision to third parties of the content: free of charge■ Usage purposes of the information acquired by disclosure request and other requests
